Conisbrough Group Practice cares for many patients in care homes, and are responsible for working with the following care homes:

Rowena House Care Home
The Dale Residential Home
Headingley Court Care Home
Headingley Park Care Home
Our clinical staff provide a weekly ward round to monitor and care for new and existing patients.
Care homes can also access urgent care from the EPC service, when we don’t have sufficient staff available for a care home visit, outside of the wad rounds.
Care homes are also encouraged to provide their own home remedies in line with Clinical Safety documentation from South Yorkshire ICB
